Many firms,for instance,have great difficulty in obtaining technical or scientific books from libraries because they are unwilling to have their names entered [as] having taken out such and such a book
很多公司向图书馆借阅科技书籍时感到很困难,因为它们不愿意让人家记下它们公司的名字和借阅的某一本书
这是一个外国大叔给我的解释 但是我不明白他说的with和that究竟是什么 求英语帝
"As" is used as "with" in this case.
"...they are unwilling to have their names entered with having taken out such and such a book...
Another way of stating this would be:
"...they are unwilling to have their names entered stating that they had taken out such and such a book.."

我其实不同意外国大叔的with说,这么说特别别扭,不能说哪儿错,就是不通顺
但是that 和as的确是可以相互替换的,都是为了解释前面的“被记下名字”.回答这些公司怎么不愿意被记下名字?（不愿意被记下有借出过哪些哪些书）
这个在中文里没有可以直接对等的表达,我也只能大概这么解释,如果还有疑问,我愿意继续解答
